The first visit will be a bit paperwork intensive if you have not already downloaded and filled out the paperwork. Please take the time to fill out the forms before your first visit. Then there will be a consultation to obtain your health and injury history.

Dr. Tim will ask you questions as to the cause and nature of your condition and other questions related to your health and symptoms. Then, there will be a thorough examination including neurologic, orthopedic and chiropractic testing to determine the cause of your problem. If further testing is required, such as x-rays, MRI's, or blood workup, then they will be ordered at that time. Dr. Tim will then give you an approximate treatment guideline based on your condition. You can either accept treatment immediately, or you can take the time to think it over before you begin treatment.

Once finished, We will collect any copay/coinsurance charges if they are listed on the card. If they are not listed, then you will be billed for the copay, deductible, or coinsurance directly. Please take the time to contact the customer service number on the back of your insurance card to verify your responsibility. If you don't understand the information that they provided, we can go over it before we begin.
